This questionnaire evolved from a Bible study that my wife and I did prior to being married. Desiring to be everything that we could for each other, and believing that Yahweh* provides the answers for fulfillment and happiness in all areas of life, we went to His Word in search of the qualities and responsibilities that we would each need to possess in order for us to be everything the other would need in our marriage.
By applying the fruits of this study for the past twenty years, we have been blessed with a successful and beautiful relationship — a rarity in today's world. Our prayer is that others may find and maintain this same marital bliss, just as Yahweh intended.
We attempted to be as exhaustive as possible. However, I encourage you to add any scriptures that you believe are appropriate.
Instructions: This questionnaire is designed to be a self-counseling manual for couples interested in improving or saving their marriage. There are six forms - three each for husband and wife. The husband's forms are marked H-1, H-2 and H-3 on the first page of each form. The wife's forms are marked W-1, W-2 and W-3.
The first form is a list of questions for you to answer about yourself; the second contains questions for you to answer about your spouse; and the third form provides a list of scriptures that corresponds with the first set of questions.
After answering the two questionnaires in your section, compare your responses with your spouse's, discuss the discrepancies and examine your answers in light of the scriptures provided for each question.
* Yahweh is the personal Hebrew name of the God of the Bible.
